Position: Registered Nurse (RN) – Home Health - PRN

Job Summary

The Home Health RN provides skilled, compassionate nursing care to patients in their homes in accordance with physician orders, agency policy, and Medicare Conditions of Participation. The RN is responsible for comprehensive assessments, developing and managing individualized care plans, and collaborating with the interdisciplinary team to ensure quality outcomes. Primary Responsibilities

Patient Care & Clinical Oversight

  • Conduct comprehensive assessments and develop individualized care plans.

  • Deliver skilled nursing services including medication management, wound care, and patient education.

  • Monitor and adjust care plans based on patient status and needs.

  • Collaborate with physicians and the care team for coordinated care.

  • Educate patients and caregivers to promote independence and safety.

Case Management & Documentation

  • Manage a primary caseload efficiently and cost-effectively.

  • Maintain accurate, timely clinical documentation in compliance with regulations.

  • Participate in IDT meetings and case conferences.

  • Supervise Home Health Aides in accordance with Medicare guidelines.

Professionalism & Communication

  • Communicate effectively with patients, families, providers, and colleagues.

  • Uphold patient rights, privacy, and confidentiality.

  • Act as a resource and mentor to peers and students as needed.

  • Participate in performance improvement, staff meetings, and continuing education.

Regulatory Compliance & Safety

  • Adhere to PA licensure laws, Medicare regulations, and agency policies.

  • Follow infection control, safety, and emergency procedures.

  • Participate in on-call rotation as assigned.

On-Call & After-Hours Care

  • Participate in evening, weekend, and holiday on-call rotation as assigned.

Secondary Responsibilities

  • Provide visits to hospice patients as needed.

  • Participate in agency-wide initiatives and committees.

  • Perform other duties as assigned.

Physical Requirements and Working Conditions

  • Able to work in varied home settings and conditions.

  • Requires walking, standing, lifting (up to 50 lbs), bending, and driving.

  • Must have reliable transportation, a valid PA driver’s license, and current auto insurance.

Qualifications

  • Graduate of an accredited RN program; BSN preferred.

  • Active RN license in the Commonwealth of Pennsylvania.

  • Minimum 1 year of nursing experience; home health or acute care preferred.

  • CPR certification required.

  • Satisfactory references and background checks.
