Job Description
We are hiring a Licensed Practical Nurse (LPN) to serve as an Assisted Living Manager for our small, residential-style assisted living facilities. This role provides clinical oversight, staff supervision, and regulatory compliance across four homes located within the same city. Ideal for an LPN who enjoys leadership and a hands-on care environment.
Responsibilities
Provide nursing oversight in compliance with Minnesota Assisted Living Licensure (144G)
Conduct resident assessments and monitor health changes
Oversee medication administration, documentation, and audits
Supervise, train, and support caregivers and medication aides
Coordinate care with providers, pharmacies, residents, and families
Ensure accurate documentation and DHS compliance
Prepare for surveys, audits, and inspections
Travel between four nearby group-home locations
Qualifications
Required:
Active Minnesota LPN license
Assisted living, long-term care, or group home experience
Knowledge of MN assisted living regulations (144G)
Strong leadership and communication skills
Valid driver’s license and reliable transportation
Preferred:
Supervisory or management experience
Experience in multi-site or group-home settings
EHR experience
CPR/First Aid certification
