Job Description
As Social Service Designee, you are delegated the administrative authority, responsibility, and accountability necessary for performing your assigned duties.
Administrative Functions
- Assist the Administrator in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility.
- Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
- Make written and oral reports/recommendations to the Administrator concerning the operation of the social service department.
- Assist in standardizing the methods in which work will be accomplished.
- Assist in making appointments for the resident/family as requested or appropriate.
- Perform charting duties, as necessary.
- Participate in completing appropriate sections of the MDS, as necessary.
- Assist in obtaining resources from community social, health and welfare agencies to meet the needs of the resident.
- Record and maintain regular Social Service progress notes indicating response to the treatment plan and/or adjustment to institutional life.
- Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
- Compile and maintain a social service directory listing available community resources.
- Compile information on discharge plans and present to appropriate committee as required.
- Coordinate social service activities with other departments, as necessary.
- Assist in the development of and participate in regularly scheduled orientation and in-service training programs in relation to the social, emotional, and medical needs of the residents.
- Attend and participate in professional activities and programs.
- Participate and assist in departmental studies and projects as assigned, or that may become necessary.
- Process resident insurance cut letters and appeals.
Experience
- Minimum of two years’ experience in long term care setting.
- Associates Degree in Social Work or LPN preferred but not required. Prior work history nd experience will be considered.
- Prior experience with insurance, discharge planning and documentation and case management preferred.
- Proof of Covid Vaccination series or qualified and approved exemption required
