Position Overview:As an LVN/LPN at Bradbury Commons, you'll be a vital member of our clinical team. You’ll deliver direct nursing care, monitor resident health, and support daily living activities in accordance with each resident's personalized care plan. This is a hands-on role that requires clinical skill, attention to detail, and a deep respect for the aging population.
- Provide direct nursing care to residents in accordance with care plans and physician orders
- Administer medications and treatments as prescribed
- Monitor residents for changes in condition and report promptly to RN supervisor or physician
- Document care and observations accurately in EMR system
- Assist with ADLs (Activities of Daily Living) when needed
- Collaborate with interdisciplinary team to ensure holistic resident care
- Participate in care plan meetings, quality improvement initiatives, and staff training
- Uphold infection control, safety, and facility policies at all times

Requirements
Qualifications:
- Current and valid LVN or LPN license in the state of employment
- CPR certification (Basic Life Support)
- Experience in long-term care or assisted living preferred
- Strong clinical and communication skills
- Compassionate, patient-centered approach
- Ability to work effectively in a team and independently
- Flexibility for shifts, including evenings, weekends, and holidays as needed
