LOCATION:                        Bronx, NY

SALARY:                           $40000.00- $45000.00 Annual


SUMMARY:

The primary purpose of this position is to support clients in developing physical, social, emotional, and daily living skills that promote independence and successful community integration. The role provides direct services including crisis intervention, behavior management, and life skills training in both individual and group settings. The position is also responsible for planning, developing, implementing, and evaluating recreational, social, and educational activities that enhance client growth and stability.

DUTIES & RESPONSIBILITIES:

  • Provide direct support to clients in their homes, program settings, and the community to promote safety, independence, and appropriate community functioning.
  • Facilitate and supervise physical, recreational, social, and educational activities.
  • Serve as a positive role model and provide consistent behavioral support.
  • Assist clients with activities of daily living, including personal hygiene, grooming, bathing, oral hygiene, and toileting, as needed.
  • Monitor clients’ needs, progress, and overall well-being; collaborate with the case management team to evaluate service effectiveness.
  • Identify potential concerns or barriers and provide input into individualized service and treatment plans.
  • Teach and reinforce daily living skills such as budgeting, grocery shopping, meal preparation, household management, and time management.
  • Support the monitoring of clients’ physical and mental health and report concerns appropriately.
  • Collaborate with multidisciplinary professionals involved in client care, serving as a support liaison when appropriate.
  • Document all client interactions, services provided, and outcomes in accordance with agency and regulatory standards.
  • Advocate on behalf of clients to obtain social service entitlements and community resources.
  • Assist clients with completing applications for benefits and other entitlements.
  • Maintain ongoing communication with supervisors, coordinators, and case managers regarding client progress and program needs.
  • Maintain contact with relevant collateral providers and attend meetings as required.
  • Participate in evening and weekend coverage as needed.
  • Perform other duties as assigned by the supervisor.

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:

Associate’s degree required, or a minimum of two (2) years of experience working with individuals affected by substance use disorders, mental health conditions, homelessness, or re-entry populations. Strong written and verbal communication skills. Demonstrated ability to provide professional and courteous customer service in person and by telephone. Excellent organizational skills with strong attention to detail. Ability to manage multiple tasks, prioritize assignments, and maintain strict confidentiality. Bilingual (English/Spanish) preferred.
