Job Summary
We are seeking compassionate and dependable Certified Nursing Assistants (CNAs) / Home Service Workers to provide high-quality, person-centered care to individuals in their homes and community settings, including assisted living and long-term care environments. In this role, you will support clients with daily living activities while promoting dignity, independence, and overall well-being. This position reports to Homecare Supervisor.

Duties & Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, eating, toileting, and mobility
- Provide medication reminders in accordance with care plans and HIPAA guidelines
- Complete accurate daily documentation of services provided
- Support clients with memory care needs, including dementia and Alzheimer’s care
- Prepare nutritious meals based on individual dietary needs and assist with feeding when necessary
- Perform light housekeeping and laundry to maintain a safe and clean living environment
- Observe, document, and report changes in client condition, behavior, or needs to the Home Care Supervisor
- Safely operate assistive devices, including Hoyer lifts and transfer equipment
- Provide emotional support, companionship, and social engagement
- Communicate effectively with clients and family members
- Follow safety protocols, including CPR and First Aid procedures
- Complete incident reports and participate in investigations as required

Requirements
- Must be at least 18 years of age
- High school diploma or GED required
- Successful completion of a state-approved CNA or Personal Care Assistant program, or comparable experience
- Ability to lift up to 50 pounds
- Valid CPR and First Aid certification (or willingness to obtain)
- TB test and physical as required
- Successful completion of a background check
- Valid driver’s license
- Valid work authorization, if applicable
- Reliable transportation (personal vehicle with insurance or other dependable means)
- Minimum of 1 year of experience in home and community-based care, assisted living, nursing homes, or hospice care
Preferred Qualifications
- Experience working with individuals with physical or developmental disabilities, autism, dementia, or Alzheimer’s disease
- Knowledge of client observation and reporting techniques
- Familiarity with HIPAA and confidentiality standards
- Experience using Hoyer lifts and safe transfer techniques
- Ability to read, write, speak, and understand English as required for the role; bilingual Spanish is a plus
