What You’ll Do:
- Deliver comprehensive, relationship-based primary care through in-home and senior community visits
- Serve as a Bloom ambassador in the San Antonio market, helping establish and grow our local footprint
- Build and manage a patient panel with an average of ~10 visits per day
- Collaborate closely with interdisciplinary teams including specialists, pharmacists, social workers, and care coordinators
- Develop trusted partnerships with assisted living communities, hospitals, clinics, and community organizations
- Use population health data and condition-specific insights to drive excellent outcomes
- Participate in ongoing education and professional development
- Travel occasionally to our Denver market for onboarding and continued training

- Strong operational support—scheduling, billing, IT, triage, and marketing are handled for you

- A proven care model with measurable quality results
- Monday–Friday schedule (3, 4, or 5 days available)
- Flexible scheduling with rotating call only 2–3 times per year
- Predictable days and manageable patient volume
- Mileage supported with a monthly auto allowance
- Competitive base salary: $225,000 – $260,000, commensurate with experience
