JOB DESCRIPTION
Pilsen Wellness Center, Inc. is a community-based not-for-profit agency that provides culturally sensitive services to a multicultural population with limited financial resources. The organizations mission has remained providing quality human services to economically disadvantaged children and families. The agency is comprised of five divisions - Mental Health, Substance Abuse, Youth Services, Early Intervention and Education which are currently supported through 17 different locations. The Wellness Inn Residential is a supervised residential program 24 hours a day for persons diagnosed with a mental illness.
We are seeking a Full-Time Mental Health Professional at our Community Service Team located at 2938 E 89th St Chicago IL 60617.
WORK SCHEDULE
The work schedule available is: Monday - Friday 8:00am - 4:00pm
POSITION SUMMARY
The Mental Health Professional (MHP) provides intensive, community-based mental health services to adults transitioning from long-term care facilities into independent living settings. The role emphasizes recovery, resiliency, and self-sufficiency by delivering individualized treatment, rehabilitation, and support services in the community. The MHP is responsible for comprehensive assessment, planning, implementation, and evaluation of services to ensure successful reintegration and ongoing stability for CST participants.
EDUCATION and/or EXPERIENCE
This individual must have a Bachelor Degree in Psychology or a related field OR 5 years’ experience in the Mental Health Field. Familiarity with Medicaid Rule 132 desired. Valid Driver's License required. Bilingual (Spanish) preferred.
COMPENSATION
$44,000-$47,000 USD per year
