We're looking for a driven and detail-oriented professional to join our team as a CHW PrEP Navigator. In this role, you'll play a vital part in helping clients navigate and access HIV testing, PrEP, care and treatment, and other community resources, helping us move forward with purpose and precision. Essential Job Duties:
- Provides confidential assessment to determine the HIV prevention needs of the clients whether test results are positive or negative; this includes the registration, and planning of prevention services.
- Provides access to biomedical prevention services (PrEP/PEP), ensuring access, follow-up, and adherence to PrEP/PEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance
- Perform HIV and other STD rapid testing
- Collaborates with project staff in advertising and media promotion of the HIV testing program, including the use of geo-social networking.
- Performs follow-up risk assessments with individuals who test positive and negative for HIV to reinforce the individuals realistic perception of risk
- Enhance care quality by providing education to providers, local organizations, and government entities to decrease bias for those we serve.

Requirements:Qualifications:
- High School Diploma or its equivalent required. Prefer college level experience in social science/public health or a related field.
- Successful completion of an HIV counseling and testing training program (or willingness to complete within 1 year of hire date).
- Experience working with culturally and economically diverse populations including LGBTQ populations.
- Valid driver’s license and reliable transportation
