About the Role
The Licensed Vocational Nurse ECM Case Manager plays a critical role in supporting members with complex medical and behavioral health needs through direct care, care coordination, and member engagement. This position works in alignment with Enhanced Care Management requirements and contracted services to promote improved health outcomes, continuity of care, and member empowerment. Caseloads may vary based on program needs and member acuity.
Key Responsibilities
Conduct blood pressure screenings and basic health monitoring for enrolled members
Provide care coordination support for ECM members with complex medical conditions
Complete medication reconciliation in collaboration with pharmacy services when available
Engage members in goal setting and encourage active participation in their care plans
Track medical and behavioral health outcome measures using a web based care management platform
Receive, review, and follow up on treatment and medication related alerts
Conduct outreach to members to assess health status, support care tracking, and reinforce care plans
Promote healthy lifestyle changes through education, coaching, and ongoing support
Collaborate with interdisciplinary team members using evidence based and client centered communication strategies
Maintain accurate, timely, and compliant documentation in accordance with ECM and Medi Cal requirements

Qualifications
Active and unrestricted Licensed Vocational Nurse (LVN) license in the State of California
One to two years of clinical nursing experience preferred
Experience working with high risk, medically complex, or underserved populations preferred
Knowledge of Enhanced Care Management, care coordination, or population health models preferred
Strong organizational skills with the ability to manage complex and deadline driven documentation
Proficiency with Microsoft Word, Excel, Outlook, PowerPoint, and Adobe Acrobat
