INTRODUCTION:
Central Neighborhood Health Foundation is a Federally Qualified Healthcare Center (FQHC) committed to the Triple Aim as described by the Institute for Healthcare Improvement. Improving the US health care system requires simultaneous pursuit of three aims: 1) improving the experience of care, 2) improving the health of populations, and 3) reducing per capita costs of health care. Preconditions for this include the enrollment of an identified population, a commitment to universality for its members, and the existence of an organization (an "integrator") that accepts responsibility for all three aims for that population.
SUMMARY:
Responsible for providing initial evaluations and individual, group, couples, and/or family therapy at the assigned CNHF clinic. In addition, therapists will provide behavioral health consultation services to the Primary Care Teams within Pediatric and Adult clinics.
ESSENTIAL DUTIES AND RESPONSIBILITIES include but are not limited to the following functions:
- Performs client orientation into the CPSP program: services provided, OB staff, program adherence, and qualifications.
- Complete all required documentation forms for the CPSP program.
- Conduct initial mental health evaluations and develop realistic treatment plans.
- Assessing clients using standardized assessment/screening tools, measures, etc.
- Provide brief, solution-focused therapy.
- Provide information and education, and referral services for identified issues.
- Provide care coordination.
- Conduct crisis assessments/triage with appropriate interventions.
- Provision of other services related to wellness
- Accurate and timely documentation of services provided in accordance with requirements
- Perform other related activities designed to provide integrated health care services
- Provide individual psychotherapy with children, adults, and families
- Provide appropriate and timely referral to a higher level of care as required
- Performs other duties as assigned
QUALIFICATION REQUIREMENTS:

EDUCATION AND/OR EXPERIENCE:
- Master’s Degree in social work from an accredited college
- Current licensure as a clinical social worker
