We’re seeking a Risk Management Coordinator to lead facility-wide risk management and patient safety efforts. In this role, you’ll investigate incidents, analyze trends, train staff, and partner with leadership to reduce risk, improve safety outcomes, and ensure compliance with regulatory and accreditation standards.

What You’ll Do

  • Coordinate and maintain the facility’s risk management and patient safety programs.
  • Conduct or oversee investigations of adverse incidents involving patients, staff, or visitors, including allegations of abuse, neglect, exploitation, and rights violations.
  • Review incident reports daily and collaborate with clinical and administrative leadership on follow-up actions.
  • Manage risk management databases and electronic incident reporting systems, ensuring accurate and timely reporting.
  • Prepare and submit detailed reports to facility leadership, corporate teams, and external agencies.
  • Aggregate and analyze incident data to identify trends, risks, and opportunities for improvement.
  • Lead or participate in root cause analyses and post-incident reviews to implement risk-reduction strategies.
  • Conduct quarterly suicide risk assessments and lead the annual Hazard Vulnerability Analysis (HVA).
  • Coordinate regulatory reviews, audits, and external agency investigations as assigned.
  • Develop and deliver training on risk management, patient safety, and regulatory requirements.
  • Serve as Chair of the Patient Safety Committee.
  • What We’re Looking For

    Education

  • Associate’s degree (required)
  • Bachelor’s degree (preferred)
  • Experience

  • Two (2) years of current risk management experience or
  • Three (3) years of current experience in a healthcare or mental health setting (required)
  • Knowledge of The Joint Commission (TJC) or other accreditation standards and patient safety principles (preferred)
