· Develop relationships with members and work on goals as outlined in the service plan. SR, D

· Complete all necessary intake paperwork with member upon initial intake and annually thereafter. SR, OC, D

· Complete and obtain all necessary discharge paperwork with member. SR, OC, D 

· Track rights restrictions and obtain quarterly signatures from appropriate parties. SR, OC, SL, D

· Upload all paperwork including intake, quarterly signatures, discharge, outside system plans, etc. to EMR. SR, OC

· Conduct monthly house meetings with members and maintain documentation of these meetings. SR, OC, D, SL, GC

· Conduct scheduled meetings/staffings with outside agencies monthly-quarterly and maintain documentation of these meetings. SR, OC, SL, GC

· Attend biweekly supervision with Case Manager Supervisor as well as weekly leadership meetings. SR, OC, D, SL, EI

· Schedule and attend annual service plan meetings with members and care team. SR, SL, OC, D, NV

· Develop service plans with new members. Update annually and as necessary. SR, OC, D

· Oversee member medication and reorder as needed alongside member. SR, OC, NV, D, SL

· Work with members on skills to complete tasks independently (i.e., medications, scheduling appointments, etc.). SR, SL, OC, GC, NV, D

· Gather necessary paperwork from referral source (i.e., PCSP, CASH, LOCUS, etc.) and upload to EMR. SR, SL, OC

· Track and complete satisfaction surveys. SR, OC

· Build and maintain relationships with outside agencies. Address concerns with supervisor as needed. SR, OC, SL, NV

· Attend job specific trainings as assigned. SR, SL, GC

· Complete and/or notify the appropriate parties to complete incident reports in EMR and IMPA. SR, OC

· Complete a SAFE-T with members when presenting or reporting suicidal ideation and communicate with appropriate parties. SR, OC, EI, NV

· Identify and utilize member’s safety plan and self-care plan when in crisis. Report to supervisor and member’s care team. SR, OC, SL, NV, EI

· Abide by The Health Insurance Portability and Accountability Act of 1996 (42 USC & 201 et seq.) 

· All other duties as assigned.

Requirements:

 A four-year degree in a Human Services related field and two years of supervised experience are necessary.
