Fellowship Hospice is seeking an experienced Part-Time Nurse Practitioner with hospice and palliative care expertise to join our interdisciplinary team. This role provides face-to-face patient care in homes and residential facilities, focusing on comfort, symptom management, and quality of life. Travel within Basking Ridge, Hackettstown, and surrounding areas is required.
Essential Functions:
- Perform comprehensive hospice and palliative care assessments addressing physical, emotional, and spiritual needs
- Manage complex symptoms including pain, dyspnea, nausea, anxiety, agitation, and end-of-life transitions
- Prescribe, titrate, and adjust medications in alignment with hospice and palliative best practices
- Lead goals-of-care discussions and advance care planning conversations with patients and families
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
- Provide education and guidance to patients, families, and caregivers regarding disease progression and comfort-focused care
- Collaborate with physicians, nurses, social workers, chaplains, and facility staff to ensure coordinated care
- Maintain accurate, timely documentation in compliance with regulatory and organizational standards
Requirements:
- Current Nurse Practitioner license in New Jersey
- Minimum of 1 year of hospice or palliative care experience required
- Demonstrated experience in symptom management and end-of-life care
- Strong clinical judgment and ability to work independently
- Excellent communication and interpersonal skills
- Willingness to travel within the service area for patient visits
- Certification in Hospice and Palliative Nursing preferred
