About the Role:
The MDS/RN at BEL Oak of Union, plays a critical role in ensuring the delivery of high-quality patient care through comprehensive assessment and care planning. This position is responsible for conducting thorough Minimum Data Set (MDS) assessments to evaluate residents' health status, functional capabilities, and care needs. The MDS/RN collaborates closely with interdisciplinary teams to develop individualized care plans that promote optimal health outcomes and regulatory compliance. This role requires meticulous documentation and adherence to federal and state guidelines to support reimbursement processes and quality improvement initiatives. Ultimately, the MDS/RN serves as a key liaison between clinical staff, residents, and families to foster a supportive and effective care environment.
Minimum Qualifications:
- Current and valid Registered Nurse (RN) license in the state of Missouri.
- Certification in Minimum Data Set (MDS) assessment or willingness to obtain within a specified timeframe.
- Strong knowledge of federal and state regulations related to long-term care and MDS requirements.
- Proficiency in electronic health records (EHR) and clinical documentation systems.
Preferred Qualifications:
- Certified MDS Coordinator credential (e.g., Certified MDS Specialist).
- Experience with quality improvement programs and regulatory survey processes.
- Advanced training in geriatric nursing or long-term care management.
- Familiarity with reimbursement methodologies such as Medicare and Medicaid.
- Excellent communication and leadership skills to effectively coordinate interdisciplinary teams.
Responsibilities:
- Perform comprehensive MDS assessments in accordance with federal and state regulations to accurately capture resident health and functional status.
- Develop, update, and coordinate individualized care plans based on assessment findings and interdisciplinary team input.
- Collaborate with nursing staff, therapists, social workers, and physicians to ensure continuity and quality of care.
- Maintain detailed and timely documentation to support clinical decision-making, regulatory compliance, and reimbursement.
- Monitor resident progress and participate in quality assurance activities to identify opportunities for care improvement.
