CFSS Agency Direct Worker

Position Overview: In-home Direct Support Professional
Model: CFSS Agency (agency-employed DSP)
Location: Minnesota, servicing individuals in their own homes and communities
Employer: Accra, LLC, licensed home health agency and CFSS provider 

Key Responsibilities

• Provide personal care and support for activities of daily living (ADLs) based on the individual’s CFSS Support Plan.
• Assist with health-related tasks, behavioral monitoring, and implementing behavior support plans, as outlined in individual plans.
• Ensure safe, constant supervision during shifts, including behavior observation, mobility assistance, and transfers.
• Complete required documentation: time sheets, EVV logs, data tracking, and communication records.
• Participate in training and follow OSHA, HIPAA, first aid, and positive behavior support protocols.

 Required Qualifications

• Must be at least 16 years old with a clean background check.
• Able to pass NETStudy 2.0 DHS background study and not be listed on OIG exclusions.
• Successful completion of Minnesota-certified PCA/CFSS training within required timeframe.

Training & Supervision Requirements

• Ongoing supervision and competency evaluations by qualified professionals per CFSS rules.

Physical & Work Environment

• Work performed in clients’ homes and community settings.
• May involve supporting individuals with complex medical, behavioral, or physical needs.
• Must be able to stand, lift, transfer, and assist continuously during shifts.
