The Youth Outreach Program Coordinator plays a critical role in advancing Three Rivers Therapy’s mission to support youth and young adults facing housing instability and homelessness. This position exists to ensure that unaccompanied youth ages 12 to 17 and young adults ages 18 to 24 have access to timely, trauma informed, and community based outreach services that promote safety, stability, and long term well-being.

As the primary staff member supporting the Office of Homeless Youth Street Outreach Services program, the Youth Outreach Program Coordinator helps bridge gaps between vulnerable individuals and essential services. The success of this role directly impacts participant engagement, continuity of care, and the organization’s ability to meet program outcomes and state expectations. Through consistent outreach, relationship building, and coordination with internal teams and community partners, this position supports meaningful connections that can change long term trajectories for youth and young adults in the Tri Cities.


Essential Duties and Responsibilities

Outreach and Engagement

  • Conduct regular community based and street based outreach throughout the Tri-Cities, specifically focusing on working with youth and young adults ages 12 to 24
  • Build trust and rapport using trauma informed, harm reduction, and culturally responsive approaches
  • Provide immediate crisis intervention and basic needs support when appropriate
  • Engage youth and young adults who may be disconnected from services and proactively reach those at higher risk of homelessness
  • Serve as a visible and consistent presence in the community to support relationship building and ongoing engagement
  • Represent the OHY Street Outreach Services (SOS) program at outreach activities, community events, and partner meetings
  • Incorporate participant feedback into outreach strategies when feasible

Case Management and Service Coordination

  • Conduct informal assessments to identify participant needs, strengths, and goals
  • Support participants in developing participant led service plans focused on housing stability, safety, education, employment, healthcare, and permanent connections
  • Provide referrals and warm handoffs to internal programs and community partners, including shelter, housing, behavioral health, medical care, education, and employment resources
  • Assist participants with diversion planning and problem solving to identify safe housing alternatives when appropriate
  • Coordinate flexible financial assistance in alignment with program and funding guidelines
  • Support participants through transitions and limited aftercare as allowed under the SOS (Street Outreach Services) program

Data, Documentation, and Compliance

  • Enter accurate and timely participant data into required data and case management systems in accordance with OHY requirements
  • Maintain complete and accurate case notes and documentation
  • Track services, referrals, outcomes, and participant engagement
  • Follow confidentiality, documentation, and data protection requirements
  • Maintain compliance with agency policies and applicable local, state, and federal regulations

Collaboration and Program Support

  • Work collaboratively with internal teams to coordinate referrals and continuity of care
  • Maintain working relationships with community partners and service providers supporting youth and young adults
  • Participate in required trainings related to trauma informed care, positive youth development, harm reduction, and cultural responsiveness
  • Support program improvement efforts and outreach strategy development as the program grows


Requirements:

 Minimum Qualifications

  • One to three years of experience working with at risk youth, young adults, and with community outreach.
  • Strong communication, documentation, and organizational skills
  • Ability to work independently in community based environments
  • Comfort working with vulnerable populations using trauma informed and strengths based approaches
  • Ability to pass required background checks

Preferred Qualifications

  • Experience with street outreach or OHY funded programs
  • Experience using case management or data tracking systems
  • Knowledge of Tri Cities area community resources and service networks
  • Bilingual skills preferred
  • Lived experience relevant to the population served

Physical and Work Environment

  • Frequent community based work
  • Ability to walk, stand, bend, and lift up to 25 pounds
  • Occasional evening or weekend work may be required
  • Exposure to variable community environments


Salary Information

$18.00 - $23.63 Hourly Wage
