Job Summary

We are seeking compassionate and dedicated Home Health Aides (HHAs) to provide high-quality in-home care to clients with diverse needs. The ideal candidate will assist with activities of daily living, monitor patient health, and support clients in maintaining their independence and well-being. Prior experience in long-term care, nursing homes, assisted living, or hospice is highly valued. Responsibilities

  • Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting
  • Monitor and observe patient conditions, reporting any changes or concerns promptly
  • Take vital signs such as blood pressure, pulse, temperature, and respiration rates
  • Support patients with mobility using equipment like Hoyer lifts or transfer belts to ensure safety during transfers
  • Provide medication reminders
  • Provide companionship and emotional support to promote mental well-being
  • Prepare meals according to dietary guidelines and assist with feeding if necessary
  • Perform light housekeeping tasks such as laundry, cleaning, and maintaining a safe environment
  • Document patient care activities accurately using EMR/EHR systems in compliance with HIPAA regulations
  • Assist with specialized care needs such as catheter care, dementia or Alzheimer's care, hospice support, or memory care
  • Support clients in the community setting by coordinating appointments


Requirements:

Skills

  • Proven experience working with seniors or individuals with disabilities in home health or long-term care settings
  • Knowledge of medical terminology and clinical procedures related to patient observation and care management
  • Ability to assist with ADLs and use medical equipment safely and effectively
  • Familiarity with EMR/EHR systems for accurate documentation of patient records
  • CPR certification and knowledge of emergency response procedures
  • Strong communication skills for interacting effectively with patients, families, and healthcare teams
  • Compassionate caregiving attitude with patience and empathy for diverse client needs
  • Ability to work independently while following care plans precisely
  • Experience working with people with developmental disabilities or in hospice environments is a plus
