Job Profile – Advanced Practice Provider (APP)

Herndon Family Medicine is seeking qualified APPs to join our team. Nurse Practitioners are a crucial part of our healthcare team, bringing a unique blend of clinical expertise and compassionate care to our medical practice. Herndon Family Medicine is an independent family medical practice, established over 30 years ago. Responsibilities & Duties

  • Conducting      comprehensive patient evaluations, including obtaining medical histories,      conducting physical exams, and treatment planning.
  • Diagnosing and managing acute and      chronic illnesses in coordination with supervising physicians, utilizing      evidence-based medicine and adhering to established clinical guidelines to      ensure optimal patient outcomes.
  • Prescribing authority, when      appropriate, including understanding pharmacology and prescribing      medications.
  • Managing overall patient care and      devising individualized care plans.
  • Educating and counseling patients      on preventive care, health maintenance and disease prevention.
  • Collaborating with physicians and      other healthcare professionals.
  • Ensuring compliance with      healthcare laws and regulations.
  • Maintaining accurate and detailed      patient records and documentation in electronic medical records system      (EMR).
  • Participating in research and      quality improvement initiatives.
  • Performing clinical procedures      within scope of practice.
  • Collaborate      with physicians, nurses, and staff to ensure seamless patient care.
Requirements:

  

Qualifications & Skills

  • Master's or Doctorate degree through      an accredited program
  • Current and unrestricted Virginia      license.
  • Certification from a recognized      certifying body (e.g., AANP, ANCC, NCCPA).
  • Minimum      of one year of clinical experience in primary care, family medicine, or      internal medicine preferred (new graduates with strong clinical rotations      may be considered).
  • Strong clinical judgment and      decision-making skills.
  • Excellent interpersonal and      communication skills.
  • Proficiency in electronic health      record (EHR) systems.
  • Commitment to continuing      education and professional development.
  • Advanced clinical training and      experience.
  • Proficiency in performing      clinical procedures.
  • Strong understanding of      healthcare laws and ethics.
  • Ability to manage complex patient      cases.
  • Strong problem-solving and      critical thinking skills.
  • Dedication to      providing high-quality patient care.
  • Ability to work independently,      while collaborating effectively with a healthcare team.
  • Up-to-date knowledge of medical advancements and      technologies.      Familiarity with state and federal regulations regarding scope of practice      for APPs.
  • Understanding      of patient privacy laws, including HIPAA compliance.
