Speech Therapist- Home Health Care

ESSENTIAL JOB RESPONSIBILITIES:

• Screens patients according to policies.
• Evaluates patients and develops appropriate plan of treatment.
• Under a physicians order, provides evaluation and direct patient treatment in accordance with an established plan of care.
• Contributes to the interdisciplinary team meeting/process.
• Consults with and trains patients, families and caregivers.
• Maintains productivity expectations on an ongoing basis.
• Documents treatment according to companies’ clinical documentation system on a timely basis.
• Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations.
• Communicates effectively with patients, families, caregivers, team members and supervisor
• Promotes positive customer relations.
• Demonstrates ability for continuous new learning to remain current in the field.
• Participates in quality improvement activities.
• Exhibits courtesy, compassion and respect to patients, families, care givers, administrators and other team members.
• Delivers services in compliance with the policies and procedures, national professional standards, state practice acts where applicable, and current accepted state of the art practice.

Education:

• Bachelor's (Required)

Experience:

• Speech therapist: 1 year (Required)
• Speech-Language Pathologists: 1 year (Required)

License/Certification:

• MA Speech therapy License (Required)

Willingness to travel:

• 25% (Preferred)
