Description:

Regional Nurse (RN) Home Care Case Manager oversees nursing standards and clinical quality. This role ensures compliance with licensing requirements, delivers high-quality client care, and provides caregiver training and competency development. The Regional Nurse reports to the VP of Operations and collaborates closely with the nursing team and community providers.


Key Responsibilities

  • Oversee nursing services, including the medication administration program, ensuring compliance and consistency across all branches
  • Conduct client nursing and medication evaluations, addressing concerns and ensuring safety and care quality
  • Train caregivers on policies, medication administration, and nurse-delegated duties, verifying competency
  • Maintain documentation and records in alignment with company protocols and state regulations
  • Review physician orders, reports, and care plans for accuracy and compliance
  • Collaborate with community providers and internal teams to coordinate care
  • Ensure adherence to in-home care licensing standards and company policies


Skills and Qualifications

  • Strong clinical assessment and case management skills
  • Ability to train and mentor caregivers effectively
  • Excellent communication, organization, and documentation skills
  • Working knowledge of electronic health records and care management systems
  • Commitment to patient safety, quality, and compliance
Requirements:

Education and Experience 

  • Current RN license in the state of which this role is based in
  • Experience with EHR or care management platforms such as Epic, WellSky, AxisCare, or AlayaCare
  • Previous experience in home care, community health, or case management preferred


Working Conditions

  • regional travel to branch offices as needed
  • Work performed in both office and client home environments


Work Location: Caring for the Cascades

Work Hours: Part Time / Hours Vary

Supervisory Responsibilities: No

Required Travel: Regular travel between branch and client homes
