REPORTS TO:
Executive Director
Perform resident health assessments, provides training/oversight to staff for medication assistance, health monitoring and assistance with health- related tasks, as allowed by regulation; coordinates resident care with third-party providers such as physicians and home health agencies.
RESPONSIBILITIES
• Promotes Harvest Home Senior Living Services mission and philosophy in daily work.
• Ensures that residents receive quality care in accordance with state and federal regulations governing long term care.
• Provides training and oversight to staff in the provision of medication assistance, health monitoring, and assistance with health-related tasks as allowed by regulation.
• Provides training and oversight to ensure appropriate infection control procedures are followed by staff.
• Conducts regular quality assurance reviews of staff documentation of medication/treatment
assistance provided.
• Conducts resident health assessments with new residents within 30 days of their move-in dates.
• Oversees or provides, coordination with resident physicians and other health care providers
• Oversees and provides coordination, in conjunction with the pharmacy used by residents.
• Coordinates and oversees the implementation of all new physician orders.
• Monitors the status of resident health conditions and initiates actions in response to changing needs; conducts re-assessments of resident health needs as indicated.
• Participates in the pre-move-in resident assessment process as requested by the administrator.
• Participates in the service planning and review process.
• Assists the administrator with conducting and/or coordinating staff in-services and training, as requested.
• Responds to resident or staff concerns and reports them to the administrator and/or others as appropriate.
• Provides health counseling/education to residents as needed.
• Reports any abuse, suspected abuse, or alleged abuse to the appropriate agency(s).
• Shares on-call to respond to staff concerns and questions according to scheduled on-call basis.
• Identifies self as an important part of the interdisciplinary team, and seeks a collaborative environment among all disciplines
• Participates in opportunities for professional growth and career enhancement.
• Performs all other duties as assigned.
QUALIFICATIONS
• Desire to work with elderly in an environment that upholds their privacy, dignity, choice, independence, and individuality.
• Working knowledge of those principles necessary to provide competent nursing care.
• Understanding of the normal dosages, actions and common side effects of most medications in common use.
• Good oral and written communication skills and the ability to work well with others.
• Effective training and supervisory skills.
• The ability to identify, address and communicate problems effectively.
• Working knowledge of and ability to conform to applicable laws, rules and regulations.
• All general staffing requirements:
• Free of communicable disease
• Able to physically/mentally perform tasks, as verified by a health screening that includes TB clearance
• Criminal record clearance and background check as required by Wisconsin Bureau of Quality Assurance
• Current First Aid certification
• Ability to read, write, and understand English
• General understanding of the needs of the elderly, those with dementia and client specific needs per Harvest Home license
• Willingness to learn
EDUCATION & EXPERIENCE
Must have an active WI RN License in good standing. Must have a passion for geriatric population and be comfortable with dementia care.
