POSITION PURPOSE |
- As part of the interdisciplinary Emergency Medicine team which also includes providers, nurses, and other healthcare professionals, the Emergency Medicine (EM) Pharmacist Lead provides care to patients through a variety of bedside clinical activities as well as indirect patient care initiatives.
- The EM Pharmacist Lead supports best practices in emergency medicine and promotes the safe, effective, and evidence-based use of medications in emergency and urgent situations, primarily within, but not limited to, the Emergency Department, a fast-paced, high-acuity setting.
- Key responsibilities include oversight of ED-related medication safety activities; medication order optimization and verification prior to administration; emergency resuscitation support; and providing expert guidance on medication dosing, medication preparation, monitoring, and toxicity management. Additional responsibilities include oversight of admission medication history activities; support in the use of high alert and procedural medications; documentation; care of boarded patients; infection control compliance and staff training, as well as quality and performance improvement (QAPI) and regulatory survey participation.

- Wisconsin Pharmacist License
- Advanced Cardiac Life Support (ACLS) & Pediatric Advanced Life Support (PALS)
- Emergency Neurological Life Support (ENLS) preferred

KNOWLEDGE, SKILLS & ABILITIES REQUIRED |
- Experience in adults and pediatric emergency medicine required
- ASHP – accredited pharmacy residency (PGY1 or PGY2 in Emergency Medicine) preferred but not required for an experienced candidate
- Board Certification in Emergency Medicine Pharmacist (BCEMP) and/or ASHP Emergency Medication Certificate (26.5 contact hours) preferred

PRINCIPLE ACCOUNTABILITIES AND ESSENTIAL DUTIES |
The Emergency Medicine Pharmacist Lead provides advanced clinical pharmacy services in the Emergency Department (ED), ensuring the safe, effective, and evidence-based use of medications for acutely ill and injured patients. This specialist serves as a key member of the interdisciplinary team, offering expertise in medication therapy, emergency response, and toxicology for adult, pediatric, and neonatal populations.
Patient Care
- Review, verify, and approve ED medication orders for accuracy, appropriateness, and safety prior to administration.
- Attend and actively participate in medical emergencies, including cardiac or respiratory arrest, trauma, and stroke.
- Conduct comprehensive home medication histories and documentation.
- Provide pharmacokinetic dosing and therapeutic drug monitoring.
- Participate and collaborate in patient-specific decision-making.
- Monitor therapeutic outcomes, lab results, adverse reactions, and make evidence-based recommendations.
- Ensure proper medication handoff when transitioning patients from ED to inpatient care.
- Provide clinical services to patients admitted in overflow areas (ie. boarded patients)
Clinical Expertise & Collaboration
- Act as a medication expert for the ED team, supporting optimal pharmacotherapy.
- Deliver drug information and clinical guidance to healthcare staff and patients.
- Collaborate with providers and nurses to provide clinical pharmacy services.
- Develop and maintain ED-specific treatment protocols, pathways, and guidelines.
Medication Therapy Management
- Ensures compliance with hospital policies, medication safety protocols, and regulatory standards
- Oversee the preparation and dispensing of high alert, emergency, and procedural medications.
- Report operational or clinical issues to management for process improvement.
Education and Training
- Maintain ACLS and PALS certification.
- Completes ongoing continuing education in emergency medicine to stay updated on new clinical developments.
- Mentor and train ED pharmacy staff, including pharmacists, interns and medication reconciliation technicians. Provide didactic and experiential education for students.
- Conduct staff in-services, emergency response training (e.g. ACLS, PALS), and other education as needed.
Quality Improvement
- Participates in quality improvement initiatives aimed at enhancing medication management practices and safety projects.
- Analyze medication use data to identify trends and implement strategies to improve safety, efficacy, and cost-effectiveness.
- Conduct medication-use evaluations and adverse event reporting.
- Represent the Pharmacy Department at Emergency Medicine Department meetings and other systemwide events such as countywide drills and FEMA training opportunities.
- Support and participate in regulatory audits and certification surveys. (e.g., Joint Commission, Center for Medicaid and Medicare, Stroke and Trauma Center certification).
Problem Solving
- Thoroughly resolves problems with appropriate and direct communication to all pertinent staff in a clear and concise manner
- Problem solving will often go beyond patient-specific issues, and may require participation in interdisciplinary teams or revising hospital or departmental policy, procedures or treatment guidelines
- Contacts supervisor for assistance when solution is beyond his/her expertise or authority
