About the role
Sylva Valley Health & Rehab is hiring an experienced RN MDS / Case Management Nurse to lead accurate assessments, strong clinical documentation, and coordinated care in our skilled nursing setting. This role is ideal for a nurse who is detail-oriented, deadline-driven, and confident managing MDS timelines while collaborating with the interdisciplinary team.

Key responsibilities
- Coordinate and complete MDS assessments accurately and on time (OBRA/PPS as applicable)
- Manage ARD scheduling, tracking, and compliance timelines
- Ensure documentation supports skilled services and reimbursement (PDPM/PPS focus)
- Participate in/lead IDT and care plan coordination
- Support case management needs: clinical updates, payer communication, continued stay reviews as needed
- Collaborate with therapy, nursing, providers, admissions, and leadership to ensure appropriate level of care and accurate documentation
- Maintain compliance with facility policies and applicable regulations
Required qualifications
- Active Registered Nurse (RN) license
- MDS experience required in a skilled nursing facility
- PointClickCare experience required
- Strong knowledge of MDS processes, care planning, and skilled documentation
- Excellent organization, communication, and time management skills
Preferred (nice to have)
- Experience with PDPM/Medicare skilled documentation and managed care reviews
- MDS certification
