Job Description

We’re looking for compassionate Medical Social Workers who are dedicated to supporting patients and families through the challenges of terminal illness. In this role, you’ll provide evaluations, counseling, and bereavement services, support care planning and coordination with interdisciplinary teams, and manage volunteer and bereavement programs.

You’ll make a meaningful impact by helping families navigate complex emotions, connecting them with resources, and ensuring care aligns with their values and needs. Key Responsibilities

  • Provide counseling to hospice patients and their families

  • Help families develop coping strategies and connect with community resources

  • Recommend therapeutic approaches for the care team

  • Conduct bereavement counseling for clients and staff

  • Educate staff and volunteers on psychosocial issues in home and hospice care

  • Facilitate interdisciplinary team conferences

  • Manage volunteer and bereavement programs

Collaborative Teamwork

This role involves working closely with our team to deliver effective, mission-focused services and contribute to a supportive and collaborative work environment.

Qualifications

  • Master’s degree in Medical Social Work from a CSWE-accredited school

  • LCSW preferred

  • Minimum of 1 year of medical social work experience

  • Hospice or home visit experience preferred

  • Experience with end-of-life care required

  • One year of experience mentoring social workers and volunteers

  • BLS certification (current and maintained)

  • Valid California driver’s license and proof of insurance

  • Ability to work with EMR and interdisciplinary teams

  • Ability to read, write, understand and communicate effectively in the English language

  • Must be able to put into practice Service Excellence skills with our patients, families, coworkers, and community
