Registered Nurse Field Hospice

Saad Hospice is seeking a full-time Registered Nurse Case Manager for our Field Hospice.  Case Managers typically work Monday-Friday 8 am to 5 pm with a call rotation.  This position requires an active RN license, reliable transportation, car insurance, a clear back ground, and a a clear drug screen.  If you are self-motivated, and love Hospice please apply.

DUTIES AND RESPONSIBILITIES: 

Maintain the standards of nursing care and follow the policies and procedures of the Hospice Agency.

Assess the patient's condition and nursing needs; prescribe appropriate actions under the direction of the physician and hospice senior leadership. 

Assess pain and symptoms and also maintain pain and symptom control.

Assess the nursing needs of the terminally ill patient.

Function as the Case Manager for the Hospice patient/family on a day-to-day basis.

Interact with patients and family in maintaining physical, mental, and emotional support.

Identify specific needs within the care setting and refer to the appropriate team member. 

Provide patient and family teaching regarding pain and symptom management, medications, dietary needs, and progression of disease process and the expected outcomes.

Support and inform patients and family regarding expectations and Hospice concept, which allows them to make an informed decision.

Ensure the patient's desires and rights are maintained.

Whenever possible, actively support the patient and family during the dying process.

Record observations timely, accurately, and concisely in accordance with Saad Hospice Documentation Standards. 

Supervise hospice LPNs and completes supervisory visits in accordance with hospice policy.

Develop and alter the hospice Plan of Care whenever indicated.

Participate as a team member in the interdisciplinary team meeting for identification of problems and problem solving.

Maintain documentation within the medical records as required by state, federal, and agency policies in a timely manner in accordance with Saad Hospice Documentation Standards.

Participate in the call rotation with the other staff members as deemed necessary.
