Position Description
Position Title: Family Caseworker
Reports to: Family Care Team Lead Non-Exempt: X
Essential Functions:
- Provide comprehensive case management services to all single residents including: intake assessment, goal setting, treatment planning, weekly case plan development, monitor progress weekly/monthly, individual money management, advocacy and referrals. Coordinate the overall interdisciplinary plan of care for the resident from intake to exit.
- Assist resident in developing support systems to meet resident needs by coordinating available services necessary to maintain self-sufficiency.
- Attend weekly meeting with resident on a consistent basis.
- Work with agencies and service providers to facilitate case coordination and information sharing. Obtain resident records from service providers.
- Provide documentation in a timely manner, which may include residents follow up, outcome evaluation, resident contact sheets and evaluations.
- Provide weekly and monthly reports.
- Provide crisis intervention when necessary.
- Communicate effectively about resident concerns with all Program Staff.
- Provide referrals and communicate with other agencies on behalf of disqualified residents.
- Attend weekly clinical meetings
- Be available to Program Specialist beyond working hours for questions or support.
- Other duties as assigned by supervisor.
Job Requirements:
- Previous experience in a not-for-profit setting
- At least two years of case management experience
- Problem Solving and reflective listening skills
- Empathy and patience towards residents
- Basic computer skills
- Ability to ascend and descend multiple stairs and/or ramps daily, multiple times a day using feet and legs; hands and arms may be used for balance (i.e., using a handrail)
- Minimum of Bachelor’s degree in social work, sociology, or psychology.
Required Education/Experience:
- Understanding of homelessness and influencing factors.
- Minimum of Bachelor’s degree in social work, sociology, or psychology.
- License Preferred.
