🧭 Position Summary
The Case Manager / Care Coordinator Level 2 assists clients in identifying needs, accessing resources, and achieving treatment goals through coordinated, person- and family-centered care.
This role includes:
Coordinating behavioral, medical, social, and educational services
Advocating on behalf of clients and families
Developing and implementing individualized treatment plans
Monitoring progress and adjusting services as needed
Collaborating with interdisciplinary providers and community partners
Maintaining accurate and timely documentation
🎓 Education & Licensure Requirements
Candidates must meet one of the following qualifications:
Bachelor’s or Master’s degree in a human services-related field (in accordance with the West Virginia Board of Social Work), OR
Licensed Psychologist (Master’s or Doctoral degree from an accredited program), OR
Licensed Registered Nurse (RN)
