Job Title: Social Worker - Care Management

Job Summary:
Carroll Hospital is seeking a compassionate and experienced Social Worker to join our Care Management team. The Social Worker will coordinate care for patients/clients, providing psychosocial support and leadership to address their needs throughout the continuum of care—from pre-admission to post-discharge. The ideal candidate will utilize community and healthcare resources effectively to ensure quality outcomes, timely interventions, and appropriate resource utilization, including managing length of stay, level of care, and denial prevention.
Key Responsibilities:

Care Coordination & Planning:
-
Gather information to determine patient/client needs and develop a collaborative care plan.

Perform psychosocial evaluations, assessing emotional, social, cultural, and spiritual aspects of patients/clients and their families.

Collaborate with healthcare teams to ensure the treatment and discharge plans address all identified needs.

Crisis Intervention & Counseling:

Provide crisis intervention and counseling services to patients/clients and their families, supporting emotional adjustment to medical circumstances.

Documentation & Compliance:

Document assessments, intervention plans, and outcomes in accordance with hospital policies and regulatory standards.

Communicate any concerns or issues to management in a timely manner.

Coordination of Treatment Plans:

Participate in rounds and patient care conferences, contributing to decision-making and treatment plan modifications.

Facilitate communication with funding sources, manage timely concurrent and retrospective reviews, and address denials and appeals.

Support Group & Case Management:

Facilitate support groups for patients and families.
-
Act as an admission coordinator by screening clients, arranging transportation, securing authorizations, and coordinating with the team and stakeholders.

Program Development & Improvement:

Participate in performance improvement initiatives and program development.

Contribute professional insights for outcome data analysis and program viability.

Participate in educational/counseling sessions for patients, families, and community members.

Team Collaboration & Leadership:

Build strong relationships with patients, families, coworkers, and the community.

Demonstrate flexibility in prioritizing work to meet patient, department, and hospital needs.

Serve as a team leader when required, ensuring effective communication and a cooperative work environment.

Qualifications:

Required:
-
Current Maryland LCSW, LCSW-C, or LMSW licensure required.

3-5 years of social work experience preferred, ideally in a healthcare or care management setting.

Knowledge of community resources and healthcare systems.
