Job Overview
We are seeking a dedicated and skilled Physical Therapist to join our dynamic healthcare team. The ideal candidate will have a strong background in patient care and a passion for helping individuals regain their mobility and improve their quality of life. This role involves working with patients across various settings, including inpatient, outpatient, and home health environments. The Physical Therapist will develop individualized care plans, implement therapeutic interventions, and monitor patient progress.

Duties

  • Conduct comprehensive assessments of patients to determine their physical therapy needs.
  • Develop and implement effective care plans tailored to each patient's specific goals and conditions.
  • Provide hands-on physiotherapy treatments, including trigger point therapy and orthopedic rehabilitation.
  • Collaborate with multidisciplinary teams to ensure coordinated patient care.
  • Educate patients and their families on treatment plans, home exercises, and lifestyle modifications.
  • Monitor patient progress and adjust treatment plans as necessary based on clinical evaluations.
  • Maintain accurate documentation of patient care using EMR systems.
  • Participate in discharge planning to facilitate smooth transitions for patients returning home or moving to other care settings.
  • Stay current with advancements in physical therapy practices and medical terminology.

Experience
Candidates should possess the following qualifications:

  • Proven experience in acute care, outpatient clinics, or home health settings.
  • Strong knowledge of anatomy and physiology relevant to physical therapy practices.
  • Familiarity with medical terminology as it pertains to patient assessments and treatments.
  • Experience working with diverse populations, including pediatrics, geriatrics, and individuals requiring specialized orthopedic care.
  • Background in home & community care is preferred but not mandatory.
