Do you believe in empowering individuals with developmental disabilities to live independently and reach their full potential? MTC is seeking a dedicated and compassionate Supported Living Coach to join our team and help individuals thrive in their homes and communities.


Key Responsibilities:

As a Supported Living Coach, you will play a vital role in helping individuals with developmental disabilities live more independently. 

  • Ensure full compliance with MTC policies, APD requirements, and all applicable regulations.
  •  Develop and implement individualized plans with the input of each person served. 
  • Maintain timely, accurate documentation in assigned case files. 
  • Provide in-person support and regular check-ins with assigned individuals. 
  • Coordinate services with Personal Support staff, as needed.  
  • Promote development of personal care, safety, daily living, and self-advocacy skills. 
  • Support individuals in achieving personal goals. 
  • Provide backup coaching services as assigned. 
  • Complete and maintain all required training and certifications.  
  • Perform additional duties as assigned.



Requirements

 

To qualify for this role, candidates must meet one of the following education/experience pathways:

  • Bachelor’s degree in education, rehabilitative science, business, or related field  OR 
  • Associate’s degree (or 2 years of college) + 2 years of direct experience with individuals with developmental disabilities  OR 
  • 1 year of college + 3 years of direct experience  OR 
  • 4 years of direct professional experience (with high school diploma or GED)

  • Person-centered thinking and strong interpersonal skills  
  • Ability to work both independently and as part of a team  
  • Proficiency in:  
    • English (spoken and written)  
    • Basic math and money management  
    • Customer service  
    • Behavioral support and adaptive skills  
    • Housekeeping and safety skills  
  • Familiarity with personal outcomes and supported living best practices  

Technical Skills:

  • Competence in Microsoft Office (Word, Excel, Outlook)  
  • Ability to document and update information in MTC software systems  

Additional Requirements:

  • Valid driver’s license, insurance, and reliable transportation  
  • Level II Background Screening/fingerprinting
  • Drug Screening  
  • Motor Vehicle Record check
