Requirements:Clinical Leadership & Operations
- Assist in managing daily nursing operations of the short-term/rehab unit.
- Supervise and support RNs, LPNs, and CNAs.
- Ensure appropriate staffing levels and skill mix.
- Monitor patient admissions, discharges, and care transitions.
- Collaborate with interdisciplinary team members (therapy, social services, physicians).
Performance Assessment & Nursing (PAN)
- Lead and monitor PAN audits and quality improvement initiatives.
- Track clinical indicators (falls, infections, rehospitalizations, wounds).
- Analyze trends and implement corrective action plans.
- Educate staff on performance standards and best practices.
- Ensure documentation accuracy and compliance with federal and state regulations.
Regulatory Compliance
- Maintain compliance with CMS and state survey requirements.
- Participate in survey preparation and plan of correction processes.
- Ensure adherence to facility policies and infection control standards
Resident Care & Customer Service
- Promote resident-centered care and high patient satisfaction.
- Address resident and family concerns promptly and professionally.
- Support safe discharge planning for short-term rehab residents.
Qualifications
- Active RN license (state-specific).
- Previous experience in skilled nursing or post-acute care.
- Supervisory or leadership experience preferred.
- Knowledge of CMS regulations, PDPM, and quality measures.
- Strong clinical assessment and organizational skills.
