Requirements:


Clinical Leadership & Operations

  • Assist in managing daily nursing operations of the short-term/rehab unit.
  • Supervise and support RNs, LPNs, and CNAs.
  • Ensure appropriate staffing levels and skill mix.
  • Monitor patient admissions, discharges, and care transitions.
  • Collaborate with interdisciplinary team members (therapy, social services, physicians).

Performance Assessment & Nursing (PAN)

  • Lead and monitor PAN audits and quality improvement initiatives.
  • Track clinical indicators (falls, infections, rehospitalizations, wounds).
  • Analyze trends and implement corrective action plans.
  • Educate staff on performance standards and best practices.
  • Ensure documentation accuracy and compliance with federal and state regulations.

Regulatory Compliance

  • Maintain compliance with CMS and state survey requirements.
  • Participate in survey preparation and plan of correction processes.
  • Ensure adherence to facility policies and infection control standards

Resident Care & Customer Service

  • Promote resident-centered care and high patient satisfaction.
  • Address resident and family concerns promptly and professionally.
  • Support safe discharge planning for short-term rehab residents.

Qualifications

  • Active RN license (state-specific).
  • Previous experience in skilled nursing or post-acute care.
  • Supervisory or leadership experience preferred.
  • Knowledge of CMS regulations, PDPM, and quality measures.
  • Strong clinical assessment and organizational skills.
