Care managers assist current and new clients with service and care plans, assisting in service coordination and handling administrative duties!
Key Responsibilities:
- Conduct initial in-home assessments to determine client needs and develop personalized care plans
- Match clients with qualified caregivers based on needs, preferences, and scheduling
Provide ongoing oversight and support to both clients and caregivers, including routine check-ins - Monitor and update care plans as needed based on client condition or feedback
- Ensure services remain compliant with Nevada non-medical home care standards and agency policies
- Maintain accurate client records and documentation in accordance with HIPAA and state requirements
- Address client concerns or incidents promptly and professionally
- Collaborate with scheduling and HR departments to ensure proper staffing coverage
- Educate clients and families on available services and set expectations clearly
Additional Information:
This is a non-clinical role. The Care Manager does not provide hands-on medical care, but focuses on service coordination, quality assurance, and ensuring compliance within the scope of non-medical home care under Nevada state law.
Requirements:Our Perfect Candidate: Qualifications
- Must have at least 1 year of experience working in healthcare or Homecare
- Must have a bachelor’s degree in business or an Associate’s degree in a related field
- Prior experience in home care, case management, or a similar role strongly preferred
- Strong communication, organizational, and problem-solving skills
- Knowledge of Nevada home care laws and regulations
- Ability to work independently and handle sensitive information with discretion
- Bilingual (Spanish/English) a plus
