JOB SUMMARY

The Wound Care Nurse Practitioner/Physician Assistant (NP/PA) provides specialized wound care services to patients in their homes, nursing homes, and assisted living facilities. This role includes comprehensive wound assessments, development and management of individualized care plans, education for patients and caregivers, and direct performance of advanced wound care procedures. The NP/PA collaborates closely with other healthcare providers to ensure optimal healing outcomes and continuity of care across multiple settings.


ESSENTIAL FUNCTIONS

- Perform thorough wound assessments at each visit and report concerns to the Medical Director.

- Provide wound care services, treatments, and advanced procedures as appropriate.

- Initiate wound care interventions based on clinical judgment and provider plans.

- Supervise and train nursing staff and medical assistants as needed.

- Perform wound care procedures as ordered by the supervising provider.

- Participate in daily stand-downs, team meetings, and case conferences.

- Maintain accurate documentation of all patient interactions in compliance with HIPAA.

- Notify the Medical Director if unable to meet a patient’s needs.

- Coordinate treatment plans with other providers involved in patient care.

- Manage schedule efficiently for productivity and patient needs.

- Comply with all infection control and safety protocols.

- Assist with orientation, education, and training.

- Adapt effectively to a variety of home and facility environments.

- Perform additional duties as assigned by the Medical Director or CMO.

Requirements:

Education

- NP: Master’s or Doctoral degree in Nursing with Nurse Practitioner specialization.

- PA: Graduate of an accredited Physician Assistant program.


Training & Licensure

- Active NP or PA license in the state of practice.

- Wound Care Certification required within 6 months of hire.


Knowledge, Skills & Abilities

- Strong clinical judgment with ability to work independently.

- Proficiency in advanced wound care principles, procedures, and infection management.

- Ability to supervise and instruct other nursing personnel.

- Strong communication and interpersonal skills.

- Culturally competent and adaptable across diverse environments.


Experience

- Minimum 2 years of clinical experience as an NP or PA (or MD/DO if applicable).

- Active patient contact within the past three years preferred.


Physical Requirements

- Frequent kneeling, squatting, reaching, twisting, climbing, and walking.

- Exposure to varying temperatures and humidity.

- Ability to assist in lifting or transferring up to 100 pounds.

- Must meet all state-required annual health screenings.

- Adequate vision/hearing with or without assistive devices.
