Position Summary:
We are seeking an experienced and detail-oriented Designated Manager to lead our comprehensive home care agency. This key leadership role will be responsible for ensuring the agency's full compliance with all relevant Minnesota Statutes, including Chapter 144A (Home Care) and the applicable sections of Chapter 245D (Home and Community-Based Services Standards). The Designated Manager will be accountable for staff training, client intake and service coordination, and for maintaining a high standard of care and regulatory adherence. The ideal candidate will have a deep understanding of integrated licensure requirements and a commitment to operational excellence.
Key Responsibilities:
Compliance & Regulatory Oversight:
- Serve as the primary Designated Manager, ensuring the agency maintains full compliance with the comprehensive home care license from the Minnesota Department of Health (MDH) and the integrated HCBS designation.
- Develop, implement, and maintain agency policies and procedures in alignment with Minnesota Statutes 144A.43 to 144A.484 and the applicable sections of 245D, particularly those related to basic support services.
- Oversee and conduct internal audits to ensure compliance with service delivery, client rights, documentation, and staff qualifications.
- Act as the liaison for all MDH and Department of Human Services (DHS) regulatory and licensing matters, including preparing for and participating in surveys and audits.
- Ensure timely and accurate reporting of critical incidents, maltreatment, and grievances to the appropriate state agencies.
- Stay current on all changes to home care and HCBS regulations and update agency practices accordingly.
Training & Staff Development:
- Design and deliver a comprehensive training and orientation program for all new and existing staff, covering MDH and 245D requirements, client rights, positive support strategies, vulnerable adult protection, and agency-specific policies.
- Maintain accurate and complete training records for all employees.
- Ensure all staff are competent and qualified to perform their assigned home care and delegated tasks.
- Provide ongoing supervision, coaching, and performance evaluations to direct care staff.
Client Onboarding & Service Management:
- Manage the client intake and onboarding process, ensuring all required assessments and documentation are completed accurately and in a timely manner.
- Collaborate with clients, families, and case managers to develop and implement Person-Centered Plans (PCPs) and Coordinated Service and Support Plans (CSSPs) that meet both home care and 245D standards.
- Oversee the provision of services to ensure they align with the client's care plan and promote their health, safety, and well-being.
- Address client and family concerns, and ensure all services are delivered with the highest level of quality and professionalism.
- Maintain accurate and complete client records in accordance with all regulatory requirements.
Qualifications:
- Bachelor's degree in a human services, healthcare, or related field is required.
- A minimum of 2-3 years of experience in a management or supervisory role within a licensed home care or 245D program is essential.
- In-depth knowledge of Minnesota Statutes Chapter 144A (Home Care) and Chapter 245D (HCBS Standards) is a must.
- Proven experience with integrated licensing (MDH comprehensive home care license with an HCBS designation) is highly preferred.
- Exceptional organizational skills and a meticulous attention to detail.
- Strong leadership, communication, and interpersonal skills.
- Proficiency in electronic health records (EHR) and Microsoft Office Suite.
- Must pass a comprehensive background study.
