Job Summary
Provide skilled, patient-focused home health care that supports safety, recovery, and independence at home.
Job Specific Duties
- Provide skilled nursing care to home health and hospice patients under the direction of the supervising Registered Nurse and in accordance with the established plan of care.
- Administer medications and perform treatments as ordered, following physician instructions and agency policies.
- Monitor and document changes in the patient’s condition, including physical, emotional, and psychosocial status, and promptly report significant findings to the RN or physician.
- Assist the RN in performing specialized procedures and maintain proper infection control and aseptic technique.
- Implement preventive and rehabilitative nursing interventions that promote safety, comfort, and independence.
- Educate patients and caregivers on treatment procedures, self-care techniques, and health management practices.
- Prepare and maintain accurate, timely clinical documentation in the electronic medical record (EMR) per CareM standards.
- Participate in interdisciplinary case conferences, staff meetings, and QAPI (Quality Assessment and Performance Improvement) activities.
- Collaborate with other disciplines—including therapy, social work, and aide services—to ensure continuity and coordination of care.
- Maintain proper care of equipment and supplies and ensure a safe environment for patients and staff.
- Respond to patient or family inquiries professionally and promptly, promoting satisfaction and trust.
- Participate in emergency preparedness, infection control, and safety initiatives as assigned.
- Provide on-call, weekend, or holiday coverage as scheduled to support continuity of care.
- Perform other duties as assigned to support patient care delivery and organizational objectives.
Other Duties:
- Provide care and services in accordance with infection control, safety, and risk prevention protocols
- Accurately document clinical interventions, care coordination, and communication in a timely manner
- Participate in interdisciplinary meetings, case reviews, and continuing education opportunities
- Maintain compliance with HIPAA and all company privacy and confidentiality standards
- Represent the organization positively and professionally in patients’ homes and community settings
- Work independently with minimal daily oversight and demonstrate initiative in completing responsibilities
- Provide courteous, timely, and professional customer service to patients, families, and partners
- Exhibit adaptability to changing responsibilities and flexibility in completing assignments
- Demonstrate consistent and effective use of organizational technology systems and tools required for the role, including communication, documentation, productivity, and data management platforms
- Adhere to all organizational policies, including job descriptions, mission, and the Employee Handbook
- Communicate and interact professionally and respectfully with others to support team goals
- Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the company

Qualifications
- Graduate of an accredited school of practical or vocational nursing.
- Current, unrestricted LPN/LVN license in the state of practice.
- Minimum of one year of clinical nursing experience, preferably in home health, hospice, or acute care.
- Current CPR (BLS) certification required.
- Demonstrated knowledge of nursing principles, clinical techniques, and infection prevention practices.
- Proficiency in electronic medical records (EMR) documentation and basic computer applications.
- Strong communication, critical thinking, and time-management skills.
- Ability to work independently while collaborating effectively as part of the interdisciplinary team.
- Commitment to confidentiality, professionalism, and compliance with CareM policies, HIPAA, and regulatory standards.
- Valid driver’s license, automobile insurance, and reliable transportation.
Physical Requirements
- Must be able to stand, walk, bend, and assist with mobility or equipment as needed. Requires the ability to perform heavy lifting and to travel locally in various weather conditions.
