Job Summary

We are looking for a licensed Speech Therapist to provide comprehensive speech, language, and swallowing therapy to patients in a home health setting. The ideal candidate will have strong clinical skills, a passion for patient care, and the ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

  • Conduct assessments to evaluate patients’ speech, language, cognitive-communication, and swallowing abilities.

  • Develop and implement individualized treatment plans based on patient needs and goals.

  • Provide therapy to improve communication skills, including articulation, fluency, voice, and language comprehension/expression.

  • Address swallowing disorders (dysphagia) through therapeutic interventions and patient/family education.

  • Document patient progress, maintain accurate records, and communicate effectively with healthcare team members.

  • Educate patients and families on strategies to support communication and swallowing goals.

  • Adhere to all state and federal regulations, as well as company policies and procedures.

Qualifications

  • Master’s degree in Speech-Language Pathology from an accredited program.

  • Current state licensure as a Speech-Language Pathologist (SLP) in [State].

  • Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred.

  • Minimum of 1 year of experience in speech therapy; home health experience is a plus.

  • Strong interpersonal and communication skills with the ability to build rapport with patients and families.

  • Ability to work independently, manage time effectively, and travel to patients’ homes.

  • Valid driver’s license, reliable transportation, and auto insurance.
