Licensed Practical Nurse (LPN)- ACT Program

The Licensed Practical Nurse provides direct assistance with daily dispensing and delivery of medications, assesses medication compliance, and assists with referrals to medical and community providers. The Licensed Practical Nurse provides community-based nursing care as part of the Assertive Community Treatment (ACT) team, supporting individuals through medication management, health education, and crisis intervention in home and community settings. The LPN works under the supervision of the team RN and in collaboration with other ACT team members. Monday - Friday 8 - 5 PM, with a rotating monthly after hours on-call obligation.
- Licensed Practical Nurse (LPN) with an active license to practice in West Virginia is required.
- Experience working with behavior health consumers is preferred.
