Job Summary:  

 

The Outpatient Behavioral Health Therapist provides quality direct and indirect patient care to individuals in the Outpatient Behavioral Health Programs. 

 

Functions and Duties:

 

  • Maintains accurate program and client documentation, including medical records. 
  • Conducts assessments, develops and implements a plan of treatment, in conjunction with members of multi-disciplinary team, communicates with referral sources and other appropriate parties regarding patients' progress, and coordinates patients' discharge planning; facilitating referrals and implementing smooth transitions.  
  • Demonstrates knowledge and skills to work with diverse group of clients based upon diagnoses and age specific needs.
  • Provides leadership for group sessions and documents client’s participation and progress daily for each client assigned to the group.
  • Provides individual therapy and family sessions.  
  • Completes a comprehensive Psychosocial Assessments. 
  • Develop an Integrated Treatment plan to include identifying the client’s short/long term goals and objectives.  
  • Document in the client's medical record all encounters with the client, family members, and collateral contacts.  Documentation will be completed timely and accurately.
  • Documents weekly the client’s ongoing progress towards the goals/objectives identified.
  • Provides appropriate discharge planning for clients to include assisting the client with setting up discharge appointments.  
  • Liaison between client, families and facilities for continuity of care and referral process.
  • Participates in Multi-disciplinary Treatment Plan Staffing Meetings.
  • Participates in the identification, implementation and evaluation of department goals and objectives.  
  • Conduct 30 day Follow-up of discharge for clients of Outpatient Behavioral Health
  • Participate in Inpatient patient staffing regarding referrals from Inpatient to Outpatient Behavioral Health, as needed
  • Assist with completing Psychosocial Assessments on inpatient unit during times of Low Census (for positions located on Touchette Regional Hospital Campus).
  • Meet with the patient while on the inpatient BHU to discuss referral to Outpatient Behavioral Health (for positions located on Touchette Regional Hospital Campus).
  • Ensure accurate billing for all encounters the EHR.
  • Maintains a safe environment monitoring continuously for any harmful contraband.  Provides crisis screening and services as needed.  
  • Attends in-services, trainings, and meetings as required.  Maintains annual competencies as assigned by the Hospital and Department.  
  • Adheres to staffing and scheduling policy.
  • Follow chain of command to resolve issues or problems identified.
  • Promotes harmonious relationships among the health care team and internal/external customers.
  • Supports and demonstrates the organization mission and philosophy through self-example.
  • Performs all other duties as assigned.

 

Minimum Qualifications:

 

Education

  • Masters in Social Work, psychology, counseling, social science or related field required
  • LCPC, LCSW, or LMFT required.

Experience

  • Two years of job-related prior in a hospital setting experience preferred.
  • Hospital experience and/or experience in a behavioral healthcare facility is preferred.

Certifications, Licenses, and Registrations

  • BLS obtained during orientation period and maintained.
  • De-Escalation and restraint training obtained during orientation period and maintained

Skills and Abilities

  • Standard office equipment (i.e., copier, facsimile machine, 10-key, etc.). 
  • Computer literate and experienced with computer programs such as Microsoft Word, Outlook 365, and Excel.
  • Knowledge of counseling theory, crisis interventions, individual, family, and group therapy modalities.
  • Knowledge of the DSM5 (Diagnostic Statistical Manual 5 and the State of Illinois mental Health and Developmental Disabilities Act.
  • Ability to handle multiple tasks and manage time efficiently.
  • Must be able to work independently with minimal supervision and exercise independent judgement and initiative.
  • Requires a concentrated level of attention with a High level of organizational skill. 
  • Ability to effectively communicate and interact with clients, visitors, and organization personnel and perform in a positive, courteous, flexible, an adaptive manner, both orally and written.

 

Physical Demands:

 

  • Sit or stand for a prolonged period of time.
  • Regular working with and viewing of computer screens.
  • Reaching by extending hands or and/or arms in any direction.
  • Finger dexterity for using computer mouse, keyboard, etc..
  • Requires regular talking and hearing.

  • Ability to adjust the eye to bring an object into sharp focus.
  • Ability to lift box of paper.
  • Must be able to wear personal protective equipment (PPE) (mask, gown, gloves) when required.

Salary Information

$27.19 - $40.79 Hourly Wage
