Position Details:
- Location: Troy office, with a hybrid schedule.
- Hours: Monday to Friday, 8:30 am to 5:00 pm.
- Additional Requirements: Weekend backup and weekly rotation of evening on-call duties (calls only).
Responsibilities:
- Receive and review case referrals.
- Assess patient needs and assign appropriate clinicians.
- Provide guidance and support to ensure quality home care services.
- Collaborate with the team to establish therapeutic goals and develop comprehensive care plans.
- Conduct quarterly record reviews and make recommendations for improvement.
- Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.
Qualifications:
- Registered Nurse with current licensure in Michigan.
- Minimum 2 years of management or supervisory experience in healthcare, preferably in home care.
- Current CPR certification.
