We're seeking a Home Care Visiting RN Supervisor to join our non-medical Home and Community Based Services team based in Monmouth and Ocean County, NJ . In this pivotal role, you'll be the advocate for the well-being of our remarkable residents . As a key player in delivering exceptional care, you'll lead in providing personalized care, overseeing clinical team members in the field, and collaborating with them to enhance the overall quality of life for our HCBS residents. Responsibilities:

  • Adhering to the Best Practices established by the Board of Nursing and CAHC Accreditation Standards
  • Open communication with other Team Members, Clients, and Family
  • Provide input regarding placing care partners appropriately with clients
  • Supervision of CHHA team members for on-campus / off-campus clients
  • Making monthly follow-up calls/visits to ensure adherence of Care Plan
  • Evaluate effectiveness of care provided by CHHAs with timely completion of 60-day skills assessments during on-site visit

Requirements:

  • New Jersey Registered Nurse (RN) License
  • BLS Certification
  • New Jersey Driver’s License, and an automobile in good working condition with appropriate insurance coverage in accordance to New Jersey laws.
  • Minimum of one year experience in health care setting, preferably home health, hospital, or long-term care
  • Ability to join on-call rotation

  • Capable of working independently and collaboratively in a multidisciplinary team
  • Proficient in electronic medical record systems and relevant software applications
