Job Summary
The Keys to Independence Permanent Supportive Housing Case Manager provides case management, recovery-oriented interactions, linkage to community resources, and monitoring of individuals with mental health, substance use, and/or co-occurring needs residing in Scattered Site Apartments in collaboration with the Milwaukee County Housing Division.
Essential Duties and Responsibilities
- Conduct regular home visits: weekly during the first 60 days of enrollment and twice monthly thereafter or more as directed by supervisor.
- Facilitate recovery- and housing-focused discussions during visits, addressing housing retention, money management, activities of daily living (ADLs), and landlord/neighbor concerns.
- Support individuals in completing Personal Recovery Crisis Prevention Plans and Individualized Service Plans in their own words.
- Provide meaningful interactions, including wellness checks and medication reminders.
- Serve as a positive role model using Motivational Interviewing, trauma-informed care, and strength-based recovery techniques to promote engagement and success.
- Collaborate with Peer Support Specialists and community case managers to meet individualized needs and link people to community resources.
- Participate in and/or coordinate monthly recovery-focused team meetings to develop supportive plans.
- Maintain regular communication with Housing Division staff to provide updates and address issues that could impact housing stability.
- Facilitate skill-building and educational groups based on individual needs.
- Maintain accurate documentation, including twice yearly outcomes and case files in accordance with agency policies and regulatory standards.
- Document all contacts and incidents in both hard and electronic files.
- Communicate incidents promptly to the Program Manager and notify the On-Call Supervisor in emergencies.
