Job Summary:
Provide care coordination, case management, behavioral health treatment intervention, and community support services to clients and families to establish and maintain client's stable functioning in the home, community and school settings. Engage in comprehensive, on-going assessments of client's ecology to determine family and system identified needs, strengths, resources, stressors, and sources of support to be used in treatment planning, providing Community Psychiatric Supportive Treatment (CPST) and Therapeutic Behavioral Service (TBS).

Essential Job Duties:
- Provides care coordination, case management and mental health intervention to Severe & Persistent Mental Illness clients providing Community Psychiatric Support Treatment (CPST).
- Assess and identify clients' medically necessary needs, empowering the client and family by enhancing their ability to identify areas of need and seek out appropriate resources with the goal of greater independence.
- Develop and implement treatment plan goals and achieve stated outcomes.
- Utilize a Wraparound approach in service delivery and promote alignment of practice with wraparound values.
- Provide evaluation services, education, intervention, and aftercare to individuals and families experiencing mental health and substance use concerns. Provide behavioral health interventions to clients; train and empower parent/guardian to provide similar interventions in the home when applicable.
- Develop and maintain cooperative working relationships with other persons and/or groups providing services to clients experiencing substance use disorders and their families, e.g. Alcoholics Anonymous, Alanon, inpatient/residential programs, court and/or probation officers, etc.
- Responsible for the vital interests of the client and family by maintaining a working knowledge of ecological/community resources. Possess knowledge of services available to client's experiencing substance use disorders and their families, and the methods of obtaining these services.

Personal Contact:
- Frequent contact with clients, staff of community agencies and the general public. Daily contact with agency staff. Must develop and maintain positive relationships with the above.
Essential Knowledge, Skills and Abilities:
Knowledge of:
- The structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
- Human behavior, diversity and cultural competence, social justice, and the impact of social determinants of health on individuals and communities.
- Treatment Planning and Intervention: You should have the ability to develop effective treatment plans based on the assessment findings. This includes formulating goals and objectives, selecting appropriate interventions and evidence-based practices, and regularly reviewing and adjusting the plan as needed.
- Care Coordination and Resource Navigation: You should have knowledge of community resources and services, including healthcare, mental health, housing, employment, financial assistance, and social support programs.
- Mental health, behavioral health, and addiction treatment resources in our service areas.
- Mental health and substance use disorders including current trends in the service area.
- Medication Assistance Treatment.
- Familiarity with relevant laws, regulations, and ethical guidelines governing behavioral health services, including HIPAA, state certification requirements, and accreditation standards.
Skills:
- Excellent communication skills are essential for building rapport with clients, collaborating with interdisciplinary teams, and advocating on behalf of your clients. Active listening, empathy, cultural sensitivity, and the ability to communicate complex information in a clear and understandable manner are crucial.
- Crisis Management: You should be skilled in identifying and managing crisis situations, including assessing for need for suicide/homicide risk assessments, domestic violence, child abuse, and other emergencies. This involves having the ability to provide immediate support and make appropriate referrals to ensure the well-being of your clients.
- Documentation and Record-Keeping: Accurate and timely documentation is vital for maintaining proper records of assessments, treatment plans, progress notes, and referrals. Familiarity with electronic health record systems and compliance with confidentiality and privacy regulations is essential.
- Professional Development and Ethical Practice: Continuously seeking professional development opportunities, staying updated on best practices and evidence-based interventions, and adhering to the ethical guidelines and standards of the profession are essential for providing high-quality care.
- Computer literacy skills, including Windows applications, Microsoft Office Suite, and basic database usage, as well as basic typing.
- Skilled in organization and attention to detail.

- Exchange information with participants and/or families in various settings regarding program plans and performance of participant.
- Work collaboratively with health care providers, community organizations and other stakeholders.
- Build relationships and supportive networks amongst the muti-county service area including a deep understanding of the resources available for mental health and addiction treatment.
- Advocate for the needs of individuals with mental health and substance use issues, raising awareness, and promoting mental health and substance use initiatives within the community.
- Complete required program documentation, follow through on assignments, and manage many deadlines. Manage time, organize, and work with multiple priorities; problem-solving and exercising sound judgment and decision making.

Environmental & Physical Requirements:
- Mobility: must be able to move independently and safely throughout the workplace, including offices, counseling rooms, and common areas.
- Mobility: must be able to drive a vehicle and complete home visits as part of this program.
- Manual dexterity: should have sufficient hand-eye coordination and fine motor skills to perform tasks such as writing, typing, operating computer systems, and handling paperwork.

- Sitting or standing: should be able to sustain a seated or standing position for extended periods while meeting with clients/external visitors, attending meetings, or performing administrative tasks.
- Emotional resilience: must have the emotional stability and resilience to work with clients/coworkers facing challenging circumstances, maintaining composure and professionalism during difficult discussions.
- Sensory sensitivity: should be sensitive to the needs of clients/coworkers and exhibit appropriate responses to their physical and emotional cues, ensuring a supportive and non-judgmental environment.
Required Qualifications:
- High School Diploma or its equivalent required.
- Ohio CDCA Certification, preferred.
- Ability to be insured on the commercial auto insurance policy based on driving record including active driver's license and personal automobile insurance and reliable transportation.
- Employment offers conditional upon completion of a background screen including fingerprinting and a drug screen
- Within the first 90 days of employment: BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation/credentialing.
